HIPAA and your rights regarding your medical record:

According to the HIPAA Code of Federal Regulations regarding medical records, an individual has a right of access to inspect and obtain a copy of protected health information about the individual in a designated record set except for psychotherapy notes and other highly sensitive information where a separate authorization is required and may also be subject to restrictions that may limit disclosure. Process for obtaining your medical record:

Medical record contents may be released to the patient and other entity, prior to patient/parent or guardian, with a signed RCBM AUTHORIZATION FOR RELEASE OF INFORMATION, and include: medication prescription and monitoring, counseling session start and stop times, the modalities and frequencies of treatment furnished, or results of clinical tests, summaries of diagnosis, functional status, treatment plan, symptoms, prognosis, and progress to date. 

Turnaround Time:

Please allow up to 30 days for the processing of your medical records request. We strive to provide you with your requested medical records in a timely manner. We understand the importance of quick access to medical information for your specific needs; this period ensures thoroughness and compliance with regulations.
